The Health Passport was made for and with people with a learning disability to help them access the health services they use.

A Health Passport can also help healthcare professionals in meeting the needs of people with a learning disability.


 

In Buckinghamshire, people with a learning disability may bring their Health Passport to their health appointments or when they have to go into hospital.

People  with a  learning disability may want to show healthcare professionals particular pages of their Health Passport.

 

For example,  people may want to tell you more on how they communicate or what medicine they are taking.

 

 

 

 

 

There is also a  professionals summary page  within the back pages of the Health Passport which can help you access key information about the person quickly.

 

With the person's permission, you can also make a note in the  Health Diary pages to  help your patient remember more about their health appointment.

You may also want to write down  important things that they need to know.

 

 

By supporting people with a learning disability with their Health Passport you are following the requirement by healthcare providers in making  reasonable changes as  directed by the Disability Discrimination Act (1995).

As a healthcare professional you are also fulfilling the expectation of the Disability Equality Duty (2006) that people with a disability should not only be treated  the same as others but that in order to achieve equal outcomes, as and when required, they should be treated differently.

Please remember that the Health Passport belongs to the person with a learning disability and is not part of their medical record. It is  patient held.
